Customer Survey Screen

After the customer has placed an order. The customer will have the option of filling out a survey regarding the order they placed. The customer has the option to either fill out the survey or dismiss it. The survey will be added to the menu display screen as another tab at the top.

The survey shows a list of the the items ordered. The customer can choose a radio button, from 1 to 5, to input their enjoyment regarding the food. When the customer is satisfied that her survey is complete, she can submit it by touching the OK button. If the survey is just ignored, the waiter will submit it when the customers leave.
